Sergio Kozciak, 44, passed away at Baystate Medical Center on April 19, 2016. He was born in Curitiba, Brazil to Francisco and Helena (Keitner) Kozciak. Sergio was employed as a Patient Care Technician for Mercy Medical Center for many years. He enjoyed watching soccer and playing poker. Sergio is survived by his parents Francisco and Helena Kozciak, his wife Sandra M. (Petit) Kozciak of Westfield and his daughters, Isabella Kozciak of Westfield and Danielle Lachance of Chicopee. Visiting hours for Sergio will be held on Friday from 4-7PM in Firtion Adams Funeral Service 76 Broad St. Westfield, MA, with a funeral home service at 7PM in the funeral home.
